This form is ONLY for re-ordering lettering for additional jerseys for a team that has already purchased from ACD Sports. We must have your order number in order to match the lettering. We may ask you to email a photo to doug@acdsports.com if we have any questions. Please include any other notes in the comment section.
The charge for the lettering will be the same as your original order. Since the costs vary per team, some are $5, some might be $15, we want you to enter a $ sign for each dollar you need to pay, then enter the # of jerseys you're ordering when you hit the Add to Cart button.
For example, if your team order lettering cost $8 per jersey, please enter 8 $: $$$$$$$$.
